Appointments, day passes, and taking payment in person

Three additions that go together more than they look like they do.

Appointments is a new module for one-on-one scheduling — a trainer, a case manager, an advisor, anyone whose time gets booked individually rather than as a group. You define appointment types, set weekly availability, and add exceptions for the days that don't follow the pattern. Members book from the portal against real availability, and reminders go out on their own ahead of time.

Day passes covers the visitor who isn't a member and isn't going to become one. They're sold, tracked, and checked in like anything else, so a drop-in shows up in your numbers instead of living on a clipboard.

Payments now work at the desk. Card-present terminals show live progress while the reader is working, keyed entry is there for the times the card won't read, and refunds are handled in the same place as the original charge. Activities, trips, meals, and rentals all use the same payment and refund path.
